Insurance Information

Payment Options

Payment is due on the day of treatment. If we accept assignment of insurance payment, the amount not covered by your insurance is due the day of treatment (your portion will be estimated). Total payment is due for first visit if we have no history with your plan in our database. If you find it more convenient to pay in full at the time of treatment, we will direct your insurance company to pay you. We accept all major credit cards, and offer contactless payments, including SamsungPay and ApplePay. You can also pay online from our website.

Dental Insurance
We welcome patients with dental insurance to our dental practice. We ask that you bring your complete insurance information at the time services are provided.

As a courtesy, we are happy to help you by submitting insurance claims. However, the office does not guarantee payment or coverage by your insurance company. We are considered an out-of-network provider, therefore, part, or all, of our services may not covered by your plan. You are responsible for payment regardless of what your insurance covers. HMOs, Medicaid and VA insurance plans will not pay us for any procedures, due to our being out-of-network. Only PPO and some Medicare Advantage dental plans can be submitted for payment.

Payment Policy
We appreciate your payment at the time services are rendered. Please be prepared to pay in full or to pay your out of pocket portion at your appointment.

A late charge will be added to your account on any unpaid balances including outstanding amounts to be paid by your insurance company. Additionally, we cannot accept responsibility for your insurance plan's delay of payment for any completed procedures, although we work hard to expedite any delay with your insurance provider.  Please call us with any financial questions or concerns. We are more than happy to help.
